That sometimes a person can be attractive in every way except that she looks like her face has been head-butted. I think it's a reference to domestic violence, and why women in those situations should somehow be considered unworthy.

I was wrong. It's just that I kind of heard the phrase more from people who didn't really show much respect towards women. Thanks for the explanation though- you should maybe put it in the OP. I wasn't the only poster confused by what you meant.
