How this will benefit the site and/or other comments:

• I propose a new method of dealing with reports in C&A. If people can't read the incredibly basic form, linking the player(s) and game(s), then no punishment should be handed out until the OP fixes it.
• If they can't be bothered to read and format correctly, why should anyone look into it for them?

The "concise description" section is where you are supposed to describe the suggestion and explain what you're asking for. The "how this will benefit the site" section is where you explain why it is a good idea. Therefore, your suggestion is rejected, because if you can't be bothered to read and format correctly, why should anyone look into it for you?

Sounds good. There really should be a standard fillable form with check boxes and automatic game links. No hunting around, everything will be immediately apparent and organized.

There's nothing you could do, outside of holding their hand and walking them through it, to make it complete idiot proof.

If they can't fill in the form they don't deserve to have it reviewed.

What they have isn't a fill form. It's just a text template of the preferred format.
